New patients please download the following forms, review them carefully, sign where appropriate, print them, and bring to your first appointment. I will answer any questions you may have at that time. You WILL NOT BE RESCHEDULED if you do not show for your initial appointment or cancel with less than 2 full business days notice to me. If your appointment is on a Monday it must be cancelled by the prior Thursday (cancel by Friday for a Tuesday appointment as Saturdays and Sundays are not business days). No exceptions to this rule so please make all necessary arrangements to be here and on time.
